Child Tracking
As a Coordinator using the ProviderTrak system you can maintain a comprehensive database of key demographic and contact information for each child in the program. The system provides for easy entry of parent, guardian, and coordinator information as well as state, county, BSU, and Medical Assistance ID numbers.
Referral and Service Tracking
The Referral and Approved Services Module allows you to enter and track referral data for services and evaluations. The user-friendly interface allows coordinators to quickly see the status of any referral and to monitor the occurrence of key events throughout the process. Through the generation of the Referral Fact Sheet, coordinators can instantly provide therapists and other appropriate parties with a document summarizing a child's services and pertinent contact information.
Services Provided
The Service Reporting Module allows for efficient entry of data related to services performed. Once entered, the services data is used to generate reports for monitoring service performance, tracking and verifying therapist activity, and for timely, accurate billing for services.
ProviderTrak includes reports as well as data exports designed to simplify the management of services. From Billing to Missed Services, providers can efficiently and effectively perform and manage all aspects of the Early Intervention service function.
